Does apple cider vinegar actually lower blood sugar?

QUICK ANSWER
Modestly, yes. A 2025 meta-analysis of seven trials in 463 people with type 2 diabetes found apple cider vinegar lowered fasting blood glucose by about 1.2 mmol/L. The reviewers rated that finding moderate certainty, and the HbA1c finding only low certainty. It is a food, not a treatment. A before-and-after HbA1c test is the honest way to see your own response.
A tablespoon in water before dinner. It is the cheapest health habit on the internet, and the most confidently recommended.
The odd thing about apple cider vinegar is that some of the research is real, and some of the most-shared research has since been withdrawn.

1. What does the evidence show?
The strongest single answer comes from a 2025 review that pooled the trials and then graded how much to trust them.
The review in Frontiers in Nutrition covered seven controlled trials and 463 people with type 2 diabetes. Fasting blood glucose fell by 21.9 mg/dL against control, which is about 1.2 mmol/L in the units UK labs use.
The reviewers also found a dose pattern. Larger effects showed up above 10 ml a day, roughly two teaspoons. They rated their confidence in the fasting-glucose result as moderate.
Two other findings matter as much as the headline one.
- HbA1c fell by about 1.5 percentage points, but the reviewers rated that finding low certainty, citing inconsistency between trials and risk of bias.
- Insulin resistance did not move. HOMA-IR, the standard research calculation, showed no significant effect.
One more caveat the authors raise themselves: five of the seven trials were run in Iran, so the findings may not transfer neatly to a UK population.
2. Why was the famous ACV study retracted?
Because the numbers in it could not be reproduced.
In March 2024, BMJ Nutrition, Prevention & Health published a small trial reporting weight and BMI loss on 5, 10 or 15 ml of apple cider vinegar a day. It travelled everywhere. On 23 September 2025, BMJ Group withdrew it.
“Concerns raised about the quality of the work, including the approach to statistical analysis of the data; implausible statistical values; the reliability of the raw data; inadequate reporting of methods; and lack of prospective trial registration.”
That retraction says nothing about the blood-sugar research above, which stands on separate trials. It does explain why the claims you read in 2024 were larger than what the evidence now supports.
3. How big is the effect, really?
Smaller than the headlines, and larger than nothing.
Read the two certainty ratings together and a fair summary emerges. Vinegar appears to blunt the glucose rise after a meal, and that shows up in fasting readings. Whether it shifts your three-month average enough to matter is far less certain.
Three things worth holding onto. The trials were run in people with type 2 diabetes, so a healthy adult starting from a normal baseline has less room to move. Vinegar is a food, not a licensed medicine. And it is not a replacement for anything a clinician has prescribed.

4. How would you know if it works for you?
Test before, wait three months, test again. That is the whole method.
The marker to use is HbA1c. It averages your blood sugar over two to three months, so it ignores the day-to-day noise a finger-prick glucose reading picks up. UK labs report it in mmol/mol. Below 42 is the normal range, 42 to 47 is the pre-diabetes band, and 48 or above sits in the diabetes range.
Because HbA1c looks back roughly three months, a retest sooner than 12 weeks will not show a fair before-and-after. Test, start the habit, hold it, retest.

5. Is it safe to drink every day?
For most people, in small diluted amounts, yes. Undiluted is the problem.
- Dilute it. Acetic acid can erode tooth enamel, so mix it into a glass of water and avoid sipping it neat.
- It can worsen reflux and cause throat irritation in people prone to either.
- Check it against your medication. If you take glucose- lowering or blood-pressure medicines, or diuretics, speak to a pharmacist first.
- More is not better. The trial doses were in teaspoons, not glassfuls.
Never stop or reduce a prescribed medicine on the strength of a food. That is a conversation for a qualified clinician.

When should I take it?
Most trials gave it with or just before meals, which fits the idea that vinegar blunts the glucose rise after eating. Dilute it in water. There is no strong evidence that one time of day beats another.
How long until I would see a change?
HbA1c reflects two to three months of average blood sugar, so allow a full 12 weeks of consistent use between a baseline test and a retest. Testing sooner will not capture the effect fairly.
