Blood Sugar Spikes and Brain Fog: Check Before You Blame Your Brain

1. Why is everyone talking about blood sugar spikes?
A ZOE Science & Nutrition episode this month put insulin and brain health in front of a large UK audience. The title did the rest of the work.
Behind the podcast sits a real piece of research. A University of Liverpool team published on 15 January 2026 looked at more than 350,000 UK Biobank participants. They used a genetic method called Mendelian randomisation, which tries to see past the usual confounders.
The headline finding: people with genetically higher glucose two hours after a glucose load had a 69 per cent higher risk of Alzheimer’s disease. Odds ratio 1.69.
2. What does high blood sugar do to the brain?
The broader link is steadier than any one study. Type 2 diabetes is one of the modifiable risk factors named in the Lancet Commission on dementia. Alzheimer’s Society puts it plainly.

The same page adds that risk rises with how long someone has had diabetes and how severe it is. Researchers at the University of Exeter set out the mechanisms in September 2025. Three come up repeatedly:
- Brain cells may struggle to use glucose for energy.
- High or swinging glucose can injure small vessels in the brain.
- Diabetes can weaken the brain’s protective barrier, which leads to inflammation.
Notice what none of that describes. A foggy Tuesday afternoon. These are slow processes measured across years, not the reason you forgot where you put your keys.
3. Does brain fog mean my blood sugar is off?
On its own, no. Brain fog is common and non-specific. Thyroid, ferritin, vitamin B12, vitamin D, sleep, stress and perimenopause all sit behind it too. Our brain fog guide walks through nine markers worth reading together.
So why check blood sugar early? Because it can drift for years without announcing itself. The NHS is direct about that.

The NHS lists the most common ones as feeling very tired, peeing more than usual, feeling thirsty all the time, and losing weight without trying to. Tiredness sits at the top. So does nothing at all.
That is the argument for measuring rather than guessing. Not because fog means diabetes. Because a quiet marker is exactly the one you would otherwise never look at.
4. Why HbA1c rather than a finger-prick glucose reading?
A glucose reading is a photograph. It tells you what your blood sugar was doing in that one moment. Skip breakfast, sleep badly or test after a run, and the photo changes.
HbA1c is closer to a three-month average. It measures how much glucose has attached itself to the haemoglobin in your red blood cells. Those cells live around three months, so the number reflects that whole window.
This is why a one-off fasting reading can look fine while HbA1c has been creeping upward. The average is harder to flatter.

5. What do the HbA1c numbers mean?
UK guidance uses three bands. NHS Scotland guidance states them clearly:
- Below 42 mmol/mol. The normal band.
- 42 to 47 mmol/mol. This range indicates prediabetes.
- 48 mmol/mol or above. The threshold a clinician uses, with confirmation, for type 2 diabetes.

A number is a measurement, not a verdict. Age nudges HbA1c upward on its own, which we cover in blood sugar levels by age. Read any result with a qualified clinician rather than against a headline.
6. Which blood test measures HbA1c?
