High Cholesterol When You Are Slim and Healthy

Cholesterol is not only about weight or diet. Slim, active people can have high readings because of inherited genes, family history, age, the menopause, or an underactive thyroid. The NHS says high cholesterol has no symptoms, so a blood test is the only way to see your numbers.

1. Can you be healthy and still have high cholesterol?
Yes, and it is more common than most people expect. A good diet and a low body weight lower your risk. They do not set the number on their own.
Part of the confusion comes from how the subject is usually discussed. Cholesterol gets framed as a food problem, so a high reading feels like a verdict on how you eat. Your liver makes most of the cholesterol in your blood. How much it makes, and how quickly it clears it again, is heavily influenced by your genes.
There is also nothing to feel. The NHS is blunt about it:
“High cholesterol does not usually cause symptoms. You can only find out if you have it from a blood test.”

2. Why do slim, active people get high readings?
The NHS lists the main risk factors, and several have nothing to do with lifestyle. It notes you are more likely to have high cholesterol if you are over 50, a man, have been through the menopause, or are of South Asian or sub-Saharan African origin. It adds plainly that it can also run in families.
Four things explain most high readings in people who look after themselves:
- Inherited genes. Some people clear cholesterol from the blood far more slowly than average.
- Age and the menopause. Levels tend to drift up with age, and often rise around the menopause as oestrogen falls.
- Thyroid function. An underactive thyroid slows how fast LDL is removed from the blood.
- Family history. A parent or sibling with a heart problem before 60 is a strong signal worth raising with your GP.
The inherited version has a name. Familial hypercholesterolaemia is a genetic condition that raises cholesterol from birth, whatever someone eats. NHS England puts its reach at approximately 1 in 250 people, about 220,000 in the UK, of whom less than 8% are currently identified. That is a lot of people running on a number nobody has explained to them.
The thyroid link is less well known. A 2018 review in Frontiers in Endocrinology reports that hypothyroidism reduces LDL receptor activity, and that 1.4% to 13% of people with raised lipids have an underactive thyroid. It is one reason a lipid result reads better alongside thyroid markers than on its own.
3. Which numbers matter beyond total cholesterol?
Total cholesterol is a blunt figure. It adds the helpful and the harmful together, which is why fit people are so often puzzled by it. Endurance training tends to raise HDL, and higher HDL pushes the total up while the underlying picture stays reassuring.
These markers say more:
- Non-HDL cholesterol. Everything except the helpful fraction, and the figure the NHS now leads on.
- ApoB. Counts the particles that carry cholesterol, rather than weighing the cargo.
- Lp(a). Largely set by your genes, and rarely measured on a standard panel.
- hs-CRP. A sensitive measure of inflammation, read alongside the lipids.

4. What should you do next?
Start by getting the full breakdown rather than one number. A single high total tells you very little. Non-HDL, ApoB and Lp(a) read together tell you a great deal more, and give you something concrete to track.
Speak to your GP if your reading is very high, or if a parent, brother or sister had a heart attack or stroke early in life. That combination is the pattern NHS services look for when considering inherited causes, and it is assessed by a clinician rather than by any single test result.
