High DHEA-S Levels UK: What a Result Above the Range Means

DHEA-S is an adrenal hormone. Your adrenal glands sit on top of your kidneys and make it, and your body uses it as a building block for testosterone and oestrogen. A high reading means your adrenals are making more of that raw material than the band for your age expects.
1. What counts as a high DHEA-S for your age?
Only the top of your own band matters. Below is the ceiling Royal Cornwall pathology publishes for each decade, in micromoles per litre.

2. Why is DHEA-S measured at all?
Mostly to look into high results. The same NHS lab says so in one sentence, and it explains why the low end gets so much more attention online than it deserves.

Three of those four reasons are about too much androgen, not too little. Virilisation means male-pattern changes in a woman. Hirsutism means unwanted coarse hair growth. Alopecia means hair loss.
DHEA-S earns its place in that list because of where it is made. It comes from the adrenal glands rather than the ovaries, so a raised figure tells a clinician which of the two to look at. 3. Does a high DHEA-S mean PCOS?
No. It is a common finding in PCOS, which is a different claim. Plenty of women with a raised DHEA-S do not have it, and plenty with PCOS have a DHEA-S inside the range.
The numbers put that in proportion. In a 2022 analysis of 648 women aged 20 to 29 with PCOS, 212 of them, or 33%, had a raised DHEA-S against their lab's own cut-off. The authors put the figure across the wider literature at about 20% to 40%.
So roughly two thirds of young women with PCOS have a normal DHEA-S. It is a supporting marker rather than a deciding one, and PCOS is recognised on a combination of findings that no single blood result settles. Two things worth saying plainly. A blood test measures markers and gives you something concrete to take to a GP. It does not diagnose any condition, and one marker outside its range does not on its own mean something is medically wrong.
4. When does a high DHEA-S need a GP?
There is a difference between a figure just over the line and a figure several times over it, and the published thresholds sit a long way up.
A review of the endocrine work-up for hirsutism puts it at 700 micrograms per decilitre, which is roughly 19 umol/L in the units a UK lab reports. Above that, the review says, a hormone-secreting adrenal growth is worth ruling out. Compare that with the 13.9 ceiling for a woman in her late twenties.
That is a clinician's threshold for investigating further, not a line you cross into a condition. A reading a little above your band is a different conversation from one at three times the band, and only a GP can tell you which you are having.
