High SHBG but Normal Testosterone: Why the Total Misleads

SHBG is the protein that binds testosterone in your blood and holds it out of use. When SHBG is high, a normal-looking total testosterone can still leave a low free level. British Society for Sexual Medicine guidance says to check SHBG and free testosterone when the total sits in the borderline 8 to 12 nmol/L band.

Those two results belong together. Read apart, the first one is reassuring and the second one is invisible. Read together, they say something the total cannot say on its own.
1. What does SHBG actually do?
It is the taxi, and it does not let the passenger out.
SHBG stands for sex hormone binding globulin. It is a protein made mostly in your liver, and it grips testosterone tightly as it travels round your blood. Testosterone held by SHBG is not available to your tissues. It is in transit, not at work.
Your total testosterone counts everything. The bound share, the loosely attached share, and the small fraction floating free. Free testosterone counts only the part that can actually do something.

2. Why can a normal total mislead?
Because a high SHBG takes a bigger cut of the same total.
Picture two men with an identical total testosterone of 14 nmol/L. One has a low SHBG, so a good share of that 14 is free. The other has a high SHBG, so most of the same 14 is locked up. Same headline number, different amount available.
This is why one number on its own can be reassuring and still be incomplete. A report that gives you a total and nothing else cannot tell you which of those two men you are.

3. What does UK guidance say to check?
The British Society for Sexual Medicine is explicit about it.
“Where the TT levels are borderline, or low to normal, particularly in obese and older men, SHBG levels should also be checked.”
TT there means total testosterone. The same guidance adds that where the total sits close to the bottom of the range, at 8–12 nmol/L, free testosterone should be checked as well.
Two numbers are quoted as supportive thresholds in symptomatic men: a total testosterone below 12 nmol/L, or a free testosterone below 0.225 nmol/L. Note the second one. A man can clear the first line and not clear the second, and a high SHBG is the usual reason.
None of that is a diagnosis you can make from a printout. These are the thresholds a qualified clinician weighs alongside your symptoms, your age and the rest of your results.
4. What pushes SHBG up?
Mostly the liver, the thyroid, and time.
- Age. SHBG drifts upward through adult life, which is why free testosterone often falls faster than total does.
- An overactive thyroid. Thyroid hormone tells the liver to make more SHBG.
- Liver conditions. Long-running liver inflammation raises production.
- Oestrogen-containing medicines. Oral oestrogen raises SHBG fairly predictably.
- Some epilepsy medicines. Carbamazepine and phenytoin are the usual examples.
- Being underweight, or eating very little. Sustained calorie restriction tends to lift SHBG.
The reverse is worth knowing too. Obesity and insulin resistance push SHBG down, which can flatter a free testosterone reading while the total looks poor.

5. What should you do next?
Get the free number, not just the total.
Free testosterone can be measured directly, but it is more often calculated from your total, your SHBG and your albumin. That is why those three markers travel together on a proper hormone panel.

SHBG usually moves when the thing driving it moves, so the honest answer depends on the cause. That is a question for a qualified clinician rather than a supplement.
