Transferrin Saturation Blood Test: What Your Percentage Actually Means

Transferrin saturation is the share of your blood’s iron-carrying capacity that is actually carrying iron, written as a percentage. Below about 20 per cent points towards low iron. Above about 45 per cent points towards iron building up. The middle is ordinary territory, and your own lab prints its range.

1. What does transferrin saturation measure?
Iron does not travel loose in your blood. It rides on a protein called transferrin. Picture a fleet of taxis moving iron from your stores to the places that need it.
An iron studies panel counts that fleet in three ways, and the three numbers are usually printed together:
- Serum iron counts the passengers.
- Total iron binding capacity, or TIBC, counts the seats.
- Transferrin saturation divides one by the other: serum iron over TIBC, expressed as a percentage.

The ratio exists because serum iron alone is restless. It moves through the day and jumps after an iron tablet. Reading the passengers against the seats steadies the picture, which is why UK labs report the percentage rather than the raw iron figure alone.
2. What does a low transferrin saturation point to?
Plenty of seats, few passengers. Demand for iron has outrun supply, and the fleet is running half empty.
A saturation below 20 per cent is the threshold used as an iron deficiency criterion in conditions that involve inflammation, such as heart failure, kidney disease and inflammatory bowel disease.
That same review explains why the percentage earns its place beside ferritin. Ferritin reflects iron stores, but it also rises with inflammation. So it can read reassuringly normal while stores are genuinely low. Where ferritin sits in the murky middle, the review asks for transferrin saturation to settle the question.

3. What does a high transferrin saturation point to?
The opposite picture. Most seats taken, and iron accumulating faster than the body is using it.
The commonly used marker is 45 per cent. A review in the British Journal of General Practice put it plainly for hereditary haemochromatosis: ferritin levels are raised and transferrin saturation is above 45 per cent in the presence of the condition.
Haemochromatosis is an inherited condition in which the body absorbs too much iron. It is worth knowing who carries the risk. The NHS says it most often affects people of white northern European background, and is particularly common where many people have a Celtic background, such as Ireland, Scotland and Wales.
4. What makes a single reading misleading?
Four things move this number, and three of them are avoidable:
- An iron tablet that morning.Serum iron jumps after a dose, so the percentage climbs with it. Leave that day’s supplement off.
- The time you sampled. Iron moves through the day. A morning sample is the one labs build their ranges around.
- A recent meal. Food shifts serum iron, so most iron studies are taken fasted or early.
- Illness or inflammation. These move iron markers in ways that have nothing to do with your stores.
None of that makes the marker weak. It means one reading is a snapshot, and a qualified clinician reads it against ferritin, your symptoms and the rest of the set. If you have been taking iron for months and nothing has shifted, this guide covers the usual reasons.
5. Which blood test measures transferrin saturation?

What is a normal transferrin saturation percentage?
Ordinary territory runs between roughly 20 and 45 per cent. Below 20 per cent is the threshold used for iron deficiency, and above 45 per cent is the marker used when iron may be building up. Ranges vary slightly by lab, so read yours against the range printed beside it.
Is transferrin saturation the same as ferritin?
No. Ferritin reflects how much iron you have stored. Transferrin saturation shows how much is in transit right now. The two answer different questions, which is why an iron studies panel reports both.
What does TIBC mean on a blood test?
Total iron binding capacity measures how much iron your blood could carry if every transferrin site were filled. It is the denominator in the saturation sum. TIBC tends to rise when iron is short, as the body makes more carrier protein.
Can a high transferrin saturation mean haemochromatosis?
It can be an early signal, but a percentage never settles it. The NHS uses a transferrin saturation test alongside serum ferritin, and confirms the picture with a genetic test arranged by a clinician. Take a raised result to your GP.
Should I stop iron tablets before the test?
Leave that day’s dose off and sample in the morning. Serum iron jumps after a tablet, which lifts the percentage and hides your usual level. If you are unsure how long to pause, ask the clinician who prescribed it.
Why is my transferrin saturation low when my ferritin is normal?
Ferritin rises with inflammation as well as with iron stores, so it can look normal while iron is genuinely short. That is the exact case where guidance asks for transferrin saturation as well. A qualified clinician reads the two together.
