That is the situation the phrase “skinny fat” describes. It is also the reason a normal BMI reassures more people than it should.

1. What does “skinny fat” actually mean?
It means the fat is in the wrong place, not that there is a lot of it.
Fat under the skin is called subcutaneous fat. It is the fat you can pinch. Fat packed around the liver, pancreas and intestines is visceral fat, and you cannot pinch it or see it. Two people at the same weight can hold very different amounts.
Researchers gave the pattern a name: TOFI, short for thin outside, fat inside. MRI scanning is what exposed it. A 2012 review in Nutrition Research Reviews, by the Imperial College team who coined the term, put the incidence at about 12% of women and 14% of men.

The idea is having a moment. ZOE’s podcast episode “The fastest way to burn dangerous visceral fat” puts it as a question: can a supermodel carry more visceral fat than a sumo wrestler? The honest answer is that body shape alone does not settle it.
2. Why does BMI miss visceral fat?
Because BMI is height and weight. Nothing in that sum knows where the weight sits.
UK guidance has already moved on this. NICE guideline NG246, published in January 2025, added waist-to-height ratio alongside BMI for anyone with a BMI under 35. You measure your waist, then divide it by your height.

The plain-English version NICE gives clinicians is simpler still. Keep your waist to less than half your height. A ratio of 0.6 or more sits in the highest band.
A tape measure is a good start. It is still a measure of shape, though, not of what is happening inside. That is where blood comes in.
3. Which blood markers track visceral fat?
Visceral fat is metabolically active tissue. It leaves a fingerprint, and five markers carry most of it.
- Triglycerides are the fat circulating in your blood, and they tend to rise as visceral fat rises.
- HDL cholesterol moves the other way, so a low HDL beside high triglycerides is the classic pairing.
- HbA1c averages your blood sugar over two to three months. UK labs report it in mmol/mol.
- hs-CRP measures low-grade inflammation, the kind visceral fat is known to drive.
- ApoB counts the particles that carry cholesterol, which is often the number that shifts first.
Triglycerides earn their place on the list. In a 2015 study in Medicine, adding a triglyceride result to age, sex, BMI and waist size explained about 13% more of the variation in measured visceral fat. The sample was small, at 51 adults, so treat it as a signpost rather than a rule.
The inflammation link is better established. Visceral fat releases signalling molecules such as interleukin-6, which prompt the liver to make more CRP. The Framingham Heart Study measured fat volumes by CT scan in 1,250 people and found visceral fat related to CRP, interleukin-6 and other inflammatory markers.
4. How do you read triglycerides against HDL?
You divide one by the other. Researchers call it the TG:HDL ratio, and it is something you can work out yourself from two results.
The ratio is used in studies as a rough stand-in for insulin resistance, which is the state where your cells respond less well to insulin. In a 2021 study of 1,608 non-obese middle-aged and older adults, it outperformed the other simple lipid ratios tested. In the obese participants it did not work at all.
One trap catches almost everyone. UK labs report lipids in mmol/L, and most of the thresholds quoted online come from US research in mg/dL. The two units convert with different factors, so the same blood gives two different-looking ratios. Our TG:HDL ratio calculator shows both.
Two honest caveats. The ratio is a research surrogate, not a NICE-endorsed test, and triglycerides need a fast beforehand to read properly. It is a prompt for a conversation with your GP, never a verdict on its own.
5. If visceral fat falls, do the numbers move?
Yes, and by more than you might expect for a modest change.
A 2025 study in Frontiers in Endocrinology followed 175 adults through a three-month dietary change and tracked visceral fat area directly.
People who lost at least 5% of their visceral fat had roughly 4.2 times the odds of a large improvement in HDL, 3 times the odds for triglycerides and 3 times the odds for HbA1c. Even losses under 5% went with better HDL.
That is the practical case for measuring. Visceral fat is the fat you cannot watch in a mirror, so the markers become your feedback loop. Test, change something, test again in three months.
6. What can a blood test not tell you?
It cannot tell you how much visceral fat you have. Only imaging can do that, and MRI or CT is not something you order for curiosity.
Blood markers describe the metabolic consequences instead. That is a different question, and often the more useful one, because it is the consequences that carry the risk.

Can a blood test measure visceral fat?
Not directly. Visceral fat volume is measured by MRI, CT or a DEXA scan. Blood markers show its metabolic effects instead, which is why triglycerides, HDL, HbA1c, hs-CRP and ApoB are read together rather than one at a time.
Can you be “skinny fat” with a normal BMI?
Yes. MRI research has repeatedly found people with a BMI in the healthy range carrying abdominal fat stores in the range seen in obesity. Researchers call the pattern TOFI, short for thin outside, fat inside.
What waist-to-height ratio does NICE recommend?
