Blood Tests for Women Over 50: What to Check Now

This guide is about the second half. What genuinely shifts after the menopause, what the NHS already checks for free, and which markers earn a place on top.
1. What changes after the menopause?
Two things matter most, and neither is a hormone reading. The first is your cholesterol.

The BHF explains why. Oestrogen helps control cholesterol and slows fat building up in your arteries, so when it falls that protection fades. A cholesterol figure from your forties may no longer describe you.
The second change runs the other way. Monthly blood loss stops, so iron stores usually recover. Many women who spent years low on ferritin, the protein that banks your iron, are no longer low at 55.
2. What does the NHS Health Check cover?
More than people expect, and it is free. The NHS says the check is for people aged 40 to 74 without certain pre-existing conditions, and that you should be invited every 5 years.
It usually includes your height, weight and waist, a blood pressure test, a cholesterol test and often a blood sugar test. That is a decent cardiovascular baseline, and after the menopause it is the part that matters most.
What it does not include: thyroid, vitamin D, vitamin B12, ferritin or any hormone. Your GP orders those when something points to them, which is not the same as having a baseline to compare against later. Our cholesterol blood test guide covers what sits behind the total figure.
3. Are hormone tests worth it at 50?
Usually not, for the question most women are asking. NICE guidance NG23 tells clinicians not to use laboratory tests to identify perimenopause or menopause in people aged 45 or over. Symptoms do that job better.
The reason is simple. Hormone levels swing week to week through the change, so one sample describes one morning rather than a stage of life. Our menopause blood test guide sets out the exceptions, including testing under 45.
There is a better use of a test at 50. Thyroid problems look a lot like menopause: tiredness, weight change, feeling cold, low mood. That overlap is worth settling, because one of the two has a treatment your GP can offer.
4. Which markers are worth adding?
Four, for most women past 50.
- The full cholesterol breakdown, not just the total figure.
- Thyroid, because its symptoms mirror the menopause.
- Vitamin D, which most of us run low on through a UK winter.
- Vitamin B12, which absorbs less readily as you get older.
Add ferritin if you had heavy periods in the run-up, or if you are vegetarian. Add HbA1c, your average blood sugar over about three months, if diabetes runs in the family or your weight has climbed.
