Cholesterol After Menopause: Why Your Numbers Changed

You are 53. The letter says your cholesterol is above target. Nothing about how you eat has changed, nothing about how you move has changed, and yet the number has. This is one of the most common surprises of midlife, and it is not a failure of willpower.
1. Why does cholesterol rise after menopause?
Because oestrogen was doing some of the work, and it stops. The British Heart Foundation puts the mechanism plainly:
“Oestrogen has a protective effect on your heart. It helps to control your cholesterol levels and reduces the risk of fat building up in your arteries.”— British Heart Foundation, Menopause and heart disease
As oestrogen falls, that help fades. LDL cholesterol tends to rise. HDL, the protective fraction, can drop or become less effective. Triglycerides often climb as well.
The NHS lists menopause alongside age and family history as a reason people are more likely to have high cholesterol. It also notes that high cholesterol “does not usually cause symptoms”, so a blood test is the only way to see it.
2. When does the shift actually happen?
Faster than most people expect, and around one specific point. SWAN followed 1,054 women through their final menstrual period and looked at whether midlife changes came from ageing or from menopause itself. On the lipid markers, the answer was menopause:
“Only total cholesterol, low-density lipoprotein cholesterol, and apolipoprotein B demonstrated substantial increases within the 1-year interval before and after the FMP, consistent with menopause-induced changes.”— Matthews et al., J Am Coll Cardiol, 2009
FMP means final menstrual period. Two things follow from that sentence.
So a woman who tested comfortably in range at 46 can be above target at 52 with an identical diet. A reading from five years ago is not a reading you can still rely on. 3. Do the targets change after menopause?
No. This is the part people get wrong. Your biology shifts, but the NHS healthy levels stay where they are.

4. Which markers are worth checking?
Total cholesterol on its own is a blunt figure. It adds the harmful and the protective fractions together, which can hide a real change. A fuller lipid picture is more useful at this stage of life.
ApoB is the one most women in midlife have never been offered. SWAN found it moving with LDL around the final period, and it counts particles rather than weighing cargo. Our ApoB guide explains the difference.

5. Does HRT change your cholesterol?
It can affect the lipid picture, and the effect depends on the type and the route. This is a prescription decision, so it belongs with your GP rather than with a guide.
What is worth knowing is the timing question. The British Heart Foundation notes that evidence points to HRT being started within 10 years of menopause and before the age of 60. Take that to your GP as a question, not a conclusion.

6. What else changes at the same time?
Less than you might expect, at least on menopause’s schedule. That SWAN paper asked a wider question than cholesterol. It tracked a range of cardiovascular risk factors through midlife and tested each one against two explanations: getting older, or the transition itself.
Only the lipid markers gave a menopause answer. The rest moved with age. That distinction is worth holding onto, because it tells you where to look and where not to.
So your lipids deserve a fresh reading around the transition. The rest of the picture, blood sugar included, still needs watching on the ordinary timetable rather than a menopause one. HbA1c is the usual companion marker, and it sits in the same panel as the lipids.

Does menopause really raise cholesterol?
The evidence points that way. SWAN found total cholesterol, LDL and ApoB rising sharply in the year either side of the final period, and the pattern followed menopause rather than chronological age. The NHS also lists having been through the menopause among the reasons people are more likely to have high cholesterol.
How much does cholesterol rise after menopause?
It varies a lot between individuals, so no single figure is honest. What is consistent is the shape of the change: a concentrated rise around the last period rather than a slow drift, and it shows up most in LDL, total cholesterol and ApoB.
What should my cholesterol be at 55?
The NHS healthy levels do not change with age or menopause: total cholesterol below 5 mmol/L, non-HDL below 4 mmol/L, and HDL above 1.2 mmol/L for women. Your GP may set a different target for you if you have another condition.
Should I test before or after my periods stop?
Both, if you can. A reading in perimenopause gives you a baseline, and one a year or two after your last period shows what the transition did. Without the first, the second is just a number with nothing to compare it to.
Do I need to fast for a cholesterol test?
Usually not. Non-HDL cholesterol was designed to be read on a non-fasting sample, which is why UK practice has moved that way. Triglycerides are the fraction most affected by a recent meal, so follow whatever instruction comes with your test.
Is a high reading after menopause something to worry about?
It is something to act on rather than worry about. A raised result is information, not a diagnosis, and cholesterol is only one input into overall cardiovascular risk. Take the numbers to your GP, who can read them alongside your blood pressure, your history and your family history.
