Citrus Bergamot for Cholesterol: Does It Work, and What to Measure?

The push traces back to one specific study: a double-blind trial published in Foodsin December 2024, now recycled through a wave of “best bergamot supplement” roundups.
The trial is real and it is reasonably well built. The problem is what happens to its numbers on the way to a product page. This guide gives you the honest read, then the part the listicles skip: how to tell whether it is doing anything for you.
1. What is citrus bergamot?
Bergamot is the citrus fruit that gives Earl Grey tea its scent. It grows almost entirely in Calabria, in southern Italy.
Supplements do not use the oil. They use an extract of the juice, rich in flavonoids with names like neohesperidin, naringin and neoeriocitrin. That mix is often sold as bergamot polyphenolic fraction, or BPF. It is the flavonoid dose, not the milligrams of extract on the front of the bottle, that the research is about.
2. What did the best trial actually find?
The study driving the current wave is a randomised, double-blind, placebo-controlled trial published in Foods in December 2024. Sixty adults aged 40 to 70 took either 375mg of bergamot extract, standardised to 150mg of flavonoids, or a placebo, every day for four months.
- LDL cholesterol fell 11.5%, from about 4.4 to 3.9 mmol/L in UK units.
- Total cholesterol fell 8.8%, from about 6.3 to 5.7 mmol/L.
- HDL rose 5.5%, but that result did not reach statistical significance.
- Triglycerides fell 16.7%, which also fell short of significance.
- Liver and kidney markers did not change, and no adverse events were reported.
Two details matter. Everyone in the trial started with raised LDL, so there was room to move. And four months is a long enough run to mean something, which is more than most supplement trials manage.
3. Why do the pooled numbers look so much bigger?
Here is where the marketing gets its bigger figures. A 2022 systematic review in Phytotherapy Research pooled 14 randomised trials. It reported an average LDL fall of 55.43 mg/dL, which is about 1.4 mmol/L.
That is roughly five times the drop the careful 2024 trial found. A gap that size is a warning sign, not a bonus. It usually means the pooled trials were small, short, or run in very different groups of people. The review's own authors were blunt about it: bergamot “might improve lipid profiles”, but its clinical efficacy “needs to be further established through higher-quality studies”.
The British Heart Foundation makes the wider point plainly. Its senior dietitian, Dell Stanford, writes that “there is not enough evidence that supplements have a consistent cholesterol-lowering effect”.
Consistent is the operative word. Averages hide how widely people respond. Some will see a clear fall in LDL. Others will see nothing.
4. Can it replace a statin?
No, and this is the part worth being firm about. A supplement that moves a number in a four-month trial has not been shown to lower anyone's risk of a heart attack or stroke. Statins have decades of outcome data behind them. Bergamot has none.

5. Which markers show whether it is working?
An 11.5% LDL claim is only knowable if you know where you started. These are the numbers that match the claim:

One marker bergamot will not touch is lipoprotein(a). It is largely inherited and does not respond to supplements. If that is what is driving your risk, no amount of bergamot will change it.
6. How do you run the before and after?
