Can You Exercise Before a Blood Test?

Light activity is fine. Hard training is not. A tough session leaks muscle enzymes into your blood, which can push liver markers, creatinine and CRP above range for days. Leave 48 hours after heavy or unfamiliar exercise, and 24 hours after a normal session. Water is fine throughout.

You train four times a week. Your test arrives on Tuesday. Nobody told you whether Monday’s session counts as preparation or as sabotage. It can be either, and the answer depends on how hard you went.
1. Does training really change your blood test results?
Yes, and by more than most people expect. Hard exercise stresses muscle fibres. Stressed fibres leak their contents into the bloodstream. Several of those contents are the exact things a routine panel measures.
The effect is largest when the session is unfamiliar. A first weights class moves your numbers far more than your usual Tuesday run. Your body treats the new stimulus as damage, because in a small way it is.
The NHS blood test page says the person arranging your test “should tell you if you need to do anything to prepare”. Exercise rarely comes up in that conversation. It should.
2. Which markers does a hard session move?
Four groups are worth knowing about before you book.

The liver enzymes are the big surprise. In a 2008 study in the British Journal of Clinical Pharmacology, fifteen healthy men did a single one-hour weights session. Their AST, ALT and creatine kinase all rose sharply, and stayed up for days:

Seven days, from one session. Bilirubin, GGT and ALP stayed normal throughout, which is the clue clinicians use. When ALT and AST climb but GGT does not, muscle is the more likely source than the liver. Our liver function test guide explains that pattern in full.
Creatinine behaves the same way. It is a waste product of muscle, so training raises it and your estimated kidney function falls with it. The UK Kidney Association treats these short-term shifts as real, noting that “a change in eGFR of up to 10% may represent variation that is not significant”.
Hormones settle faster. A 2021 review in the Scandinavian Journal of Medicine & Science in Sports found that testosterone and cortisol jump straight after interval training, dip below baseline a couple of hours later, then return to baseline by 24 hours.
3. How long should you rest before a blood test?
Helvy asks you to leave 48 hours after a heavy or unfamiliar session, and 24 hours after a normal one. Those are our preparation rules, set on the cautious side.
- Heavy weights, a race, or a new class— leave 48 hours.
- Your usual run, ride or gym session— leave 24 hours.
- Walking, stretching, yoga— no rest needed.
- Water— keep drinking it, right up to the test.
Water matters more than people think. Training dehydrates you, and a dry sample is the usual reason a finger-prick kit fails at the lab. Drink normally the night before and the morning of.

4. What if you already trained?
Do not panic, and do not skip the test. A normal session the day before changes a thyroid, vitamin D or B12 result very little.
If it was a hard session and the panel covers your liver or kidneys, rebook. Two days costs you nothing and gives you a number you can trust. That is far cheaper than a repeat test and a fortnight of worry.
If you go ahead anyway, say so. Note the session when you submit your sample. A raised ALT reads very differently once someone knows a leg day sat in front of it. On an NHS full blood count, the same logic applies to a briefly raised white cell count.
5. How to check your own markers
Most people who train never see these numbers until something prompts a GP test. Checking them while you feel well is what makes a later change readable.

Can I go to the gym the day before a blood test?
A normal session is usually fine, and 24 hours is enough for most panels. Skip it if the session would be heavy, unfamiliar or much longer than usual. In that case leave 48 hours.
Does exercise raise liver enzymes?
It can, sharply. ALT and AST sit in muscle as well as the liver, so a hard weights session releases them. One study found the rise lasted at least 7 days. GGT usually stays normal, which helps a clinician tell muscle from liver.
Does exercise affect kidney function results?
Temporarily, yes. Creatinine is a muscle waste product, so hard training pushes it up and your estimated eGFR down. It settles within a day or two. Retesting after rest is how the picture gets clarified.
Should I stop training before a testosterone test?
