High Creatine Kinase Levels UK: What a Raised CK Actually Means

Every Helvy guide is written by our health editors, then checked by a qualified clinician before it goes live and re-checked as the science moves. QUICK ANSWER
Creatine kinase is an enzyme your muscles release when muscle cells are damaged. Most UK labs call 40 to 320 U/L normal for men and 25 to 200 U/L for women. Hard training is the commonest reason CK runs high, and it can stay up for a week afterwards.
Not sure which markers you need? Build your test →Your CK came back at 900. The report has flagged it in red. You feel fine, apart from legs that have not forgiven you for Saturday.
That combination is ordinary. CK is a damage signal, and damage is what a hard session does on purpose.
1. What counts as a high CK?
Two NHS labs publish the same adult figures, split by sex. Your own report prints the range your result was measured against, which is the one that counts.
| Lab | Men | Women |
|---|---|---|
| Gloucestershire Hospitals | 40 to 320 U/L | 25 to 200 U/L |
| South Tees Hospitals | 40 to 320 U/L | 25 to 200 U/L |
Men sit higher because they carry more muscle, so more enzyme circulates at rest. Gloucestershire lists strenuous exercise, an injection into a muscle, bruising, crush injury and surgery among the things that push it up. Guidance from North Bristol NHS Trust adds that CK values run slightly higher in some ethnic groups.
So the size of the number matters more than the flag beside it. A CK just over the line and a CK of 6,000 are different situations.
2. Did you train hard in the week before the test?
Ask this first. It explains most raised CK results in people who feel well. Muscle releases CK when fibres are damaged, and a heavy session damages fibres. That is how training works.
The scale surprises people. The North Bristol protocol puts intensive exercise at up to roughly 30 times the upper limit of normal. On a 320 U/L ceiling, that is a CK in the thousands from training alone.
The timing catches people out too. Fifteen healthy men did one hour of weightlifting in a 2008 British Journal of Clinical Pharmacology study. Their CK was still raised seven days later, and so were two liver numbers.
Our guide on exercise before a blood test covers how long to leave between a session and a sample.
3. Why does a raised CK make your ALT look bad?
This is the part that sends people down a liver rabbit hole. ALT, short for alanine aminotransferase, is known as a liver enzyme. Mostly it is one. It also sits in skeletal muscle, in smaller amounts.
Damage a lot of muscle and ALT leaks out beside the CK. Your liver is fine. Your report suggests otherwise.
“Intensive muscular exercise, e.g. weightlifting, should also be considered as a cause of asymptomatic elevations of liver function tests in daily clinical practice.”
Pettersson et al., British Journal of Clinical Pharmacology, 2008
Asymptomatic means you feel nothing. In that study, ALT and AST stayed up for at least seven days. Bilirubin, GGT and ALP stayed normal throughout.
That contrast is the useful bit. Liver trouble usually disturbs more than the two enzymes muscle also makes. So if your ALT is up, your CK is up, and you trained hard beforehand, muscle is the likelier source. A repeat after a rest week settles it.
Our guide to high ALT levels walks through the other reasons that number runs high, and the ALT blood test guide explains what it measures.
4. What about muscle aches on a statin?
Aching muscles are a known statin side effect, and they are a common reason a GP orders a CK at all. NHS Specialist Pharmacy Service advises checking CK where there are persistent muscle symptoms, not as a routine test for everyone starting one.
“Stop existing statin or do not initiate statin if creatine kinase (CK) greater than 5 times ULN.”
NHS Specialist Pharmacy Service, statins monitoring
ULN means upper limit of normal, so five times the top of your lab's range. The same guidance says to re-measure after 7 days. If CK is still above five times, the statin is not restarted. If it has come down, it can restart at a lower dose.
One nuance matters if you lift or work physically. The guidance suggests specialist advice where a baseline CK is extremely high because of a physical occupation or rigorous exercise. A bricklayer does not start from the same number as a desk worker.
None of this is yours to decide alone. Do not stop a prescribed statin on the strength of a web page. Our guide on statin muscle aches and CoQ10 covers the evidence.
5. When is muscle pain urgent rather than a testing question?
Get urgent medical help if severe muscle pain comes with dark urine, the colour of cola or strong tea. That colour can come from myoglobin, a muscle protein that spills into urine when a lot of muscle breaks down at once.
The word for that is rhabdomyolysis, which means muscle breaking down fast enough to threaten the kidneys. The North Bristol protocol treats CK above 5,000 mIU/L as the level suspicious of it, and notes myoglobin colours urine red-brown above 300 mg/L.
That is an A&E conversation, not a test to buy. The same goes for worsening muscle weakness, and for pain after a crush injury or a long spell lying immobile.
6. Does Helvy test creatine kinase?
No. Creatine kinase is not part of any Helvy panel, and we are not going to pretend otherwise on a page about it. If you need a CK measured, that request belongs with your GP.
What we do carry is the set of numbers a raised CK gets confused with. Thyroid & Vital Organs reads ALT and ALP alongside creatinine, eGFR, albumin, total protein, your thyroid hormones and your lipids. General Energy & Wellness reads CRP, an inflammation marker.
Creatinine deserves a warning of its own. It is a kidney marker, not creatine kinase, though the names get swapped constantly. It also rises with muscle mass and creatine supplements. We cover that in does creatine raise creatinine and in the creatinine blood test guide.
If you are not sure which panel fits, the build-my-test tool walks you through it in about a minute.
READY TO TEST?

Helvy's home blood tests report ALT with plain-English context, so a figure lifted by a hard week never sparks a needless panic. Samples are processed by UKAS-accredited UK labs.
CK is measured on a statin only when muscle symptoms are unexplained, which blood tests for statins sets out.
Frequently asked questions
What is a high creatine kinase level in the UK?
Above 320 U/L for a man and above 200 U/L for a woman at Gloucestershire Hospitals and South Tees. Check the range printed on your own report, since that is what your result was measured against.
Can exercise cause a high CK?
Yes, and it is the commonest reason. North Bristol NHS Trust guidance puts intensive exercise at up to roughly 30 times the upper limit of normal. A four-figure CK after a heavy session is not unusual.
How long does CK stay raised after a workout?
Longer than most people expect. In a 2008 study of fifteen healthy men, a single one-hour weightlifting session left CK raised for at least seven days. Leave a gap before a repeat test.
Does a high CK mean liver damage?
No. It points the other way. Muscle damage raises ALT and AST as well as CK, which is why a training block can make liver numbers look wrong. In that 2008 study, GGT, ALP and bilirubin all stayed normal.
Should I stop my statin if my CK is high?
That is your prescriber's call, not yours. NHS Specialist Pharmacy Service guidance stops or withholds a statin above five times the upper limit of normal, then re-measures after 7 days. Speak to your GP first.
When is a raised CK an emergency?
Severe muscle pain with dark, cola-coloured urine needs urgent medical help the same day. So does worsening muscle weakness. Those are hospital questions, not reasons to order a private test.
Does Helvy measure creatine kinase?
No, CK is not part of any Helvy panel. Thyroid & Vital Organs carries ALT and ALP with your kidney markers, and General Energy & Wellness carries CRP. A CK request is one for your GP.
