Free T3 Blood Test: The Marker Your GP Skips

Free T3 is the active thyroid hormone your cells actually use. UK labs report a range of roughly 2.4 to 6.0 pmol/L, and the exact numbers differ by laboratory. NHS protocol only adds free T3 to your sample when your TSH comes back below the reference range, so most people never see it measured.

Your thyroid test came back normal. You are still exhausted, still cold, still not yourself. Then you read that T3 is the hormone that does the actual work, and notice it was never on your report. Here is why, and what the number means.
1. What does a free T3 blood test measure?
T3 is triiodothyronine. It is the thyroid hormone that actually works inside your cells, setting the pace of your metabolism, your heart rate and your body temperature.
Your thyroid makes very little of it directly. Most T3 is converted from T4, the storage hormone, in your liver, kidneys and other tissues. So T4 is the reservoir and T3 is the delivery.
Most T3 in your blood is stuck to carrier proteins and cannot be used. A free T3 test measures only the unbound fraction, written FT3. That is the part available to your cells, and it is why labs report free T3 rather than total T3.
2. What is a normal free T3 level?
There is no single UK range. The numbers depend on the assay each laboratory uses, and the gap between them is wider than most people expect. Two published NHS ranges show it:

Look at the bottom end. A free T3 of 3.0 pmol/L is comfortably normal in London and below range in Bristol. Same blood, different verdict.

3. Why won’t my GP test my T3?
Because the national guideline tells the lab not to, unless one specific condition is met. NHS thyroid testing runs on a protocol called cascading. TSH is measured first, and further markers get added to the same sample only when TSH is abnormal.
NICE sets out exactly when free T3 joins in:
“If the TSH is above the reference range, measure free thyroxine (FT4) in the same sample. If the TSH is below the reference range, measure FT4 and free triiodothyronine (FT3) in the same sample.”

Read it twice and the logic lands. Free T3 is only added when TSH is below range, which points to an overactive thyroid. If your TSH is normal or high, free T3 is never measured.
That is deliberate, and for most people it works. It also means the marker is invisible to anyone whose TSH sits inside the range while their symptoms carry on. Getting free T3 measured then usually means testing privately.
4. What do high and low free T3 mean?
Free T3 means little alone. Paired with TSH, patterns appear:

NHS labs mainly use free T3 to investigate a suspected overactive thyroid, as North West London Pathology sets out. A test measures biomarkers, though. It does not diagnose anything by itself, and only a qualified clinician can read your result in context.
The last row catches people out. A low free T3 with a normal TSH is usually your body dialling down conversion during illness, heavy dieting or prolonged stress. It is a response, not a thyroid fault. Low iron and low B12 mimic the same fatigue, and both show on a full blood count.
5. How to check your free T3 at home

Frequently asked questions
What is the difference between T3 and free T3?
Total T3 counts all the triiodothyronine in your blood, including the large share bound to carrier proteins and unavailable to your cells. Free T3 counts only the unbound portion. Pregnancy and the contraceptive pill change your carrier proteins, so free T3 gives the steadier reading.
Can free T3 be low when TSH and T4 are normal?
Yes. It is usually a sign your body has slowed T4 to T3 conversion, which happens during illness, very low calorie intake or prolonged stress. It is a response rather than a thyroid problem. Persistent symptoms deserve a qualified clinician, not a repeat test.
Should I test reverse T3?
Reverse T3 is not part of NHS thyroid testing and NICE NG145 does not recommend it. Helvy does not measure it either. The evidence that it changes what anyone should do is thin, so we report free T3, free T4 and TSH instead.
Do I need to fast for a free T3 test?
No, fasting is not needed. Stop high-dose biotin supplements two days beforehand, because biotin interferes with many thyroid assays. If you take levothyroxine, take your sample before your dose.
