Low Creatinine Levels UK: What a Result Under the Range Means

Kidney trouble pushes creatinine up, not down. A low number is almost always telling you something about your body, not your kidneys. Here is what it measures, why yours sits under the range, and the one thing worth checking.
1. What counts as a low creatinine?
Creatinine is a waste product. Your muscles make it as they work, and your kidneys filter it out into your urine. It is measured in micromoles per litre, written µmol/L.
There is no single UK cut-off. Labs set their own bands, and most now split them by sex.

So a 52 is low at Royal Cornwall and comfortably normal at North Bristol. Read the range printed on your own report. A chart you found online is a rough guide at best.
2. Why is your creatinine low?
Muscle, most of the time. Creatinine production tracks how much muscle you carry, so less muscle means less creatinine in the blood. Leeds Teaching Hospitals puts it plainly in its pathology handbook:
“Creatinine is related to muscle mass so women and children tend to have lower levels than men.”

The usual reasons a result lands below the range:
- A small or light frame. Less muscle to shed creatinine from.
- Pregnancy. Blood volume rises and the kidneys filter faster, so the number drops.
- Getting older. Muscle falls away from midlife onward unless you train.
- A long illness or a spell in bed. Muscle goes quickly when you stop using it.
- Eating little or no meat. Dietary creatine feeds the pool your muscles draw on.
- Advanced liver disease. Uncommon, and it comes with other abnormal liver numbers.
Pregnancy shifts the goalposts enough that some labs publish separate bands. Royal Liverpool lists 25 to 80 µmol/L at 12 weeks against 50 to 90 outside pregnancy. A 30 would be flagged on the standard range and is ordinary at 12 weeks.
Creatine supplements do the opposite, which surprises people. They nudge creatinine up rather than down, as does creatine raise creatinine explains.
3. What does a low creatinine do to your eGFR?
This is the part worth knowing. Your eGFR is not measured. It is calculated from your creatinine, your age and your sex. Feed a low creatinine into that sum and the eGFR comes out high.
For most people that is fine, because the low creatinine is genuine and so is the good kidney function. The exception is someone who has lost a lot of muscle. Their creatinine can sit inside the range, or below it, while their kidneys are doing less than the number suggests.
Leeds makes the same point from the other side. Creatinine, it says, is “inversely related to the GFR and so there can be a considerable fall in the GFR before serum creatinine levels begin to rise”. 4. When is a low creatinine worth following up?
On its own, rarely. In company, sometimes. Take it to a qualified clinician if the low creatinine arrives with any of these:
- Weight or muscle you did not mean to lose.
- A low albumin or a low total protein on the same report.
- Liver numbers that are also out of range.
- Tiredness or weakness that has not shifted in weeks.

5. Which blood test includes creatinine?

Yes. Royal Liverpool publishes 25 to 80 µmol/L at 12 weeks against 50 to 90 outside pregnancy. A result that looks low on the standard range can be normal for the stage you are at.
Can I raise my creatinine level?
Building muscle raises it over months, because there is simply more tissue making it. That is not a reason to chase the number. Creatinine is a measurement, not a target.
Should I see a GP about a low creatinine?
Speak to a qualified clinician if it comes with unplanned weight loss, a low albumin, abnormal liver numbers or tiredness that will not shift. On its own it rarely needs action.
