Urea is a waste product. Your liver makes it when it breaks down protein, and your kidneys clear it into urine. Two of those three steps have nothing to do with how well your kidneys filter, which is exactly why the number moves so easily.

2.5 to 7.8 mmol/L. Gloucestershire Hospitals NHS Foundation Trust pathology publishes that as the reference range for blood urea, and most UK laboratories now use the same figures. There is no adult age chart. One range applies from 18 upward.

One point of history is worth knowing. Before 2012 many UK labs used 2.5 to 7.0 mmol/L. A national harmonisation project moved the top of the range to 7.8. If you are comparing an old result to a new one, check which range each was measured against.
2. Why urea is not only a kidney number
Because two ordinary things push it up before your kidneys get a say. The first is how much protein you have eaten, since protein is the raw material. The second is how much water is in your blood. Dry out a little and everything dissolved in your blood reads more concentrated, urea included.

The same charity is clear about the fix. Urea is a good indicator of kidney function alongside creatinine, not on its own. Creatinine comes from muscle at a steadier rate, so it is less pushed around by what you ate and drank. That is why a lone raised urea is a prompt to look at the pair, not a finding.
3. What a high urea usually means
Start with the everyday explanations, because they are the common ones:
- Not drinking much, or a hot day, or a hard training session.
- A high-protein diet, or a heavy dose of protein shakes.
- A stomach bug that left you short of fluid.
- Some medicines, including diuretics and steroids.
- Reduced kidney filtering, which shows up in creatinine and eGFR too.
The pattern is what matters. Urea up on its own, with a normal creatinine and a normal eGFR, usually points at fluid or food. Urea up together with a raised creatinine and a lower eGFR is the combination that describes filtering, and that one belongs with a GP.
If you want a clean reading, drink normally for a couple of days before testing, and do not load protein the night before. Then retest. A repeat taken properly hydrated answers the question far faster than worrying about the first result.
4. What a low urea means
A urea below 2.5 mmol/L worries people far more than it usually should. The common causes are a low-protein diet, drinking a lot of water, and pregnancy, which lowers it as a normal change.
Because the liver makes urea, a persistently low result can also reflect how the liver is working. That is worth mentioning to a clinician if it keeps showing up, especially alongside liver markers such as ALT and albumin. One low reading after a big week of water and salad is not the same thing.

The marker is urea, reported in mmol/L. No fasting is needed, though normal hydration matters more here than on most markers.

Urea comes from protein you eat, so diet and hydration move it. Creatinine comes from muscle at a steadier rate, so it tracks filtering more reliably. Both are cleared by the kidneys, which is why they are reported together and read as a pair.
Do you need to fast before a urea blood test?
No. Fasting is not required for urea. Drinking normally in the day or two beforehand matters more, because dehydration is the single most common reason a healthy person sees a mildly raised result.
